At first, pilots told flight attendants not to evacuate passengers, National Transportation Safety Board (NTSB) Chairman Deborah Hersman said. The evacuation of a plane that crashed on Saturday at the San Francisco airport did not begin until 90 seconds after the wreckage had skidded to a stop, an official has said.
The evacuation began when an attendant saw a fire outside the plane, she said.
The crash of Asiana flight 214 killed two and injured about 180 people.
At a press conference on Wednesday, Ms Hersman also confirmed some of the evacuation slides had inflated inside the aircraft as it bounced along the ground, injuring a flight attendant and possibly hindering passengers' escape.
She has said two of the pilots - the flying pilot and the training pilot - were both in their respective roles in the Boeing 777 for the first time on Saturday, despite being otherwise experienced fliers.
